Issuance of copies of Medical Records

Important information - read before you apply

  • Requests for copies of medical records are accepted via e-Form or directly at the Registry.
  • A copy of the medical records will usually be prepared within 1 business day of the request.
  • Collection of the documentation is possible at any Luxmed Medical Center Branch. A copy of the medical records is not sent by regular mail to the address indicated by the Patient, due to the impossibility of verifying the identity of the recipient and securing the confidentiality of the shipment.
  • A copy of the medical records is sent by email to the email address indicated by the patient in the application, which includes a trusted signature in a PDF document.
  • A copy of the medical records will be issued to the Patient, the Patient's legal representative or an authorized person on the basis of verification of an identity document.
  • An authorized person is a person with notarized authorization or a person designated by the Patient in a statement filled out personally by the Patient at the Direct Registration.
  • In accordance with current regulations, concerning the protection of personal and medical data, other types of authorization will not be honored, due to the impossibility of verifying their authenticity.
  • A fee is charged for the issuance of copies of medical records in accordance with Article 28 of the Law of November 06, 2008 on Patient Rights and Patient Ombudsman.
